Forme is a recovery tracking platform for aesthetic medicine. We help providers manage recovery tracking after procedures. This policy explains what information we handle and why.
What We Collect
We only collect what's necessary to make the platform work:
For you (if your provider enrolled you): Your name, phone number, and the recovery photos you choose to upload. Your provider's office provides your name and phone number when they enroll you. Photos are only uploaded by you, voluntarily.
For providers: Your name, email, and practice information when you create an account.
That's it. We don't track your browsing behavior, we don't build advertising profiles, and we don't collect anything beyond what's listed above.
What We Do With It
Your information is used for exactly two purposes:
Sending you recovery check-in messages (via SMS) on behalf of your provider.
Securely storing your recovery photos so your provider can access them through the platform.
What We Don't Do
We do not sell your data. Not to advertisers, not to data brokers, not to anyone.
We do not share your phone number or opt-in information with third parties for marketing.
We do not use your photos for anything other than showing them to your provider — unless you explicitly consent otherwise.
Text Messages
Forme sends SMS messages on behalf of your provider for post-procedure recovery follow-ups.
You'll receive 1–6 messages over a 3-month recovery period, depending on your provider's protocol.
Standard message and data rates from your carrier may apply.
Reply STOP at any time to opt out. You'll get a confirmation and we won't text you again.
Your data is encrypted in transit and at rest. Access is restricted to you and your provider. We use trusted, secure infrastructure to store everything.
Your Choices
You can delete your photos directly from your account at any time — they are permanently removed from our servers. If you want your entire account removed, email connect@myforme.health. If you opt out of text messages, we stop texting you — your photos and account remain accessible unless you delete them.
