Forme is a recovery tracking platform for aesthetic medicine. These terms cover how the platform works and what to expect when you use it.
Forme helps aesthetic medicine providers track recovery after procedures. Providers use the platform to manage follow-ups. You receive text messages with a link to upload recovery photos at key milestones. That's the core of what we do.
Your provider's office enrolls you in Forme during your intake process. After your procedure, you'll receive a small number of text messages (typically 1–6 over three months) with a link to upload a recovery photo. Uploading is always voluntary — you're never required to participate.
Your photos are only accessible to your provider — they are never made public unless you give explicit permission for your provider to use them.
You're responsible for getting proper consent from the people you enroll before enrolling them in Forme. The platform gives you tools to do this, but the consent itself happens at your practice during intake.
You own your photos. By uploading them, you give Forme and your provider permission to securely store them within the platform for recovery tracking. Your photos are only accessible to your provider — they are never made public unless you give explicit permission for your provider to use them. When you delete a photo from your account, it is permanently removed from our servers.
Forme is a technology tool — not a medical provider. We don't give medical advice, diagnose conditions, or recommend treatments. Always talk to your provider about your recovery.

We may update these terms as the platform evolves. If we make material changes, we'll notify you via email or through the platform before they take effect. Continued use of Forme after changes means you accept the updated terms.
